Using Predictive Analytics to Reduce Heart Failure Hospitalizations at Advocate Aurora

Advocate Healthcare leveraged its extensive data platform to create predictive models that identify patients who are at elevated risk of going to the hospital in the next 90 days. It has been using one of these models to identify heart failure patients for a targeted outpatient care management program. Early results have shown that patients in this program are going to the hospital less often than patients in a control group.
