The changing economics of healthcare are stimulating the emergence of innovative approaches to clinical integration (physician alignment and the improved coordination of care). Providing care that is accountable and population focused requires not only a financial imperative, but also technology tools to enable these new care models focused on maintaining safe and effective care and patient satisfaction with the care experience. We discuss emerging model and technologies that coordinate care at the transactional level as well as those that aggregate data for advanced analytics. And we focus on observations from interactions with multiple stakeholders, from providers to health plans and beyond.
