Community Collaboration Address Social Determinants to Reduce Costs and Improve Health

These services are not yet fully quantified or qualified from an integrated health system-community based organization approach to value-based reimbursement.  This session focuses on innovative efforts underway in upstate New York as part of the DSRIP 1115 waiver to support the transition from fee for service to value-based care and payment, with focus points on how community agencies can be positioned for collaboration with larger networks of care.
