This session outlines how Advocate worked on the creation, validation and deployment of a 30-day all cause readmission predictive model.  It also highlights how the model has been used within the context of an accountable care environment to improve care coordination and reduce readmissions.  Also reviewed is the next phase of analytical model work, including predicting the best transition location for a patient.
