SI Readmissions Collaborative Kickoff

Erica Drazen, ScD, Senior Advisor, Scottsdale Institute, and former VP, Global Institute for Emerging Practices, CSC, leads a discussion about the purpose of the Collaborative, the starting point and the plans for future work. All SI members are welcome, but to be an active collaborator you need to be currently involved in interventions to reduce readmissions (or plan to do so in the near future) and be willing to share your results.
