Carol Mayer, Network Project and Operations Manager, supports Social Determinants of Health, Diversity Equity and Inclusion (DEI), and Health Equity for Academic Affairs. She is responsible for the continued coordination, development, budget management, and implementation of operational processes across the network.
With an extensive network of stakeholders she identifies, develops, implements, and administers SDoH operating procedures while fostering an innovative environment to assist in meeting the key strategic initiatives. She tracks progress across multiple initiatives, identifies bottlenecks, and risks who prioritizes needs of traditionally underserved populations by prompting and incorporating diverse perspectives.
She supports the monitoring/evaluating of the SDoH screening and response interventions. She partners with analysts in the development of operational dashboard for screenings, referrals, issues, and outcomes. She supports the expansion of screening into additional practices in primary care across the network with future expansions for the inpatient setting. She coordinates the design of Electronic Medical Record (EMR) tools to facilitate referrals to community resource specialists/community healthcare workers. She works closely with Accountable Care Organization team to ensure screening meets optimal healthcare outcomes.
She is a graduate of University of Massachusetts at Amherst who has the privilege of working in this position and being surrounded by those dedicated to improving healthcare outcomes for our local community.
