Coordinator for Comprehensive Primary Care Plus (CPC+) Mercy Family Medicine
Tamra Lavengood holds the position of Clinical Performance Coordinator for Centura Health Physicians Group and is the CPC+ Coordinator for Mercy Family Medicine in Durango, CO. She holds two Bachelor degrees in Biology and Nursing and a Master’s degree where she earned certification as a Pediatric Nurse Practitioner.
Tamra has been a RN for 36 years; has worked in the inpatient setting both on the floor and the ICU; in the outpatient clinic setting; as a Nurse Supervisor; Nurse Manager; Clinical Nurse Specialist; Advanced Practice Nurse; Quality Specialist; and Program Development and Coordinator. Tamra developed and implemented a Pediatric Patient Classification System for Denver Health and Hospitals for her Master’s thesis which captured the patient’s acuity and care requirements that related to staffing requirements.
In her current role she was the lead in the development of the Mercy Risk Stratification Tool that risk stratifies patients to target care management resources to the highest risk population.
Tamra was recently awarded the CPC+ Person of the Year for 2019. She presents both locally and nationally on healthcare transition that focuses on making Primary Care Primary.
