Theresa Osborne, MD

Dr. Terri Osborne brings over 25 years of experience as a board-certified internal medicine physician. She currently serves as Medical Director for Quality and Special Populations for Spectrum Health Medical Group in Grand Rapids, Michigan.
Dr Osborne earned her medical degree at the Medical College of Ohio in Toledo and completed her residency program at Akron City Hospital in Akron, Ohio. She recently completed the Master of Health Care Delivery Science at Dartmouth College, Hanover, New Hampshire .
Since 2005, she has been instrumental in work related to Delivery Redesign and Quality Improvement. This work included the establishment of a shared medical appointment program for patients with diabetes, embedding high-risk RN care managers in primary care practices , development and evaluation of an enhanced access model of care, and formation of multidisciplinary teams to work on quality improvement in bone health, heart failure, and anticoagulation. She has spoken in 9 states about her work .
Dr Osborne was physician lead for two CMS research grants—the COMPASS enhanced depression management CMMI grant, co -directed by Health Partners Institute for Education and Research and Institute for Clinical Systems Improvements in Minnesota, and the Michigan Primary Care Transformation Project directed by the Michigan Department of Community Health. Dr Osborne’s team, in partnership with West Michigan Heart, a large cardiology group, implemented the regional SHMG Anticoagulation Clinic . In addition, Dr. Osborne partnered with TransforMED , leading her own site to NCQA-Patient Centered Medical Home (PCMH) Level 3 Recognition.
Dr. Osborne enjoys wilderness backpacking, travel, and reading in her spare time.
